Hepatic duct obstruction caused by stones leading to biliary tract infection

Ms. Gia, 56 years old, experienced pain in the right lower quadrant spreading to her back, along with fever, chills, and tremors. The doctor determined that stones were blocking the liver duct, leading to abscesses that caused a biliary tract infection.

A magnetic resonance imaging (MRI) scan of Ms. Dinh Thi Gia’s abdomen revealed dilated intrahepatic bile ducts on both sides, with the right hepatic duct measuring 1.5 cm and the left 2.5 cm. Stones were found in the patient’s left hepatic duct, extending to the common hepatic duct, along with sludge and bile sludge forming branches of liver segments.

On June 7, Dr. Vo Ngoc Bich from Tam Anh General Hospital stated that Ms. Gia required surgery to address her severe biliary tract infection and alleviate pain, infection, and prevent complications. Due to previous open surgeries, the procedure was complex as there was significant omentum attached to the abdominal wall and the liver area.

During the surgery, doctors removed multiple stones and abscesses from the liver ducts, leading to the removal of segments 2 and 3 of the liver, representing 30% of the organ. A common bile duct tunnel was created to aid in the removal of stones and drain the bile duct, ensuring a successful outcome for the patient.

After the surgery, Ms. Gia recovered well, regained her appetite, and was discharged from the hospital after six days. Dr. Bich mentioned that nearly all stones were removed, eliminating the need for medication upon discharge. Patients who undergo stone removal surgery have a high risk of recurrence and should undergo regular check-ups every 3-6 months.

Intrahepatic bile duct stones, known as hepatolithiasis, are common in Asia and can lead to severe infections if left untreated. Dr. Bich recommends preventive measures such as a healthy diet, hygienic practices, and regular health check-ups for individuals with a history of bile duct stones to maintain overall well-being.
