WellSpan Health Medical Records

Under the Health Insurance Portability and Security Act (HIPAA), you have the right to view your medical record and request changes to it. One way to access your health information quickly and easily is through the MyWellSpan portal. This portal allows you to manage your care from any device at any time.

To request a copy of your medical record through MyWellSpan, simply go to the Sharing section in the menu and select Request Medical Records. Provide the necessary information to receive your medical record electronically. If you are unable to use MyWellSpan, you can use a paper form called Authorization to Use or Disclose Health Information to request your medical record.

If you need to make changes to your medical record, the best method is through your MyWellSpan account. By navigating to the Document Center and selecting Requested Records & Record Amendments, you can submit a request for a change in your medical record with detailed information. Alternatively, you can use an online or paper form, such as the Patient Requested Amendment Form, to mail or email your request for a change in your medical record.

WellSpan Health also provides other medical record forms for specific requests, such as the Next of Kin Verification Form for deceased loved ones or the 21st Century Cures Act Request Form for requests to access, exchange, or use electronic health information. These forms are available for different purposes and should be used accordingly.
