Environmental Scan and Literature Review of Holistic Care Coordination in Kansas

Holistic care coordination (HCC) is a patient-focused approach that takes into account the physical, emotional, and social aspects of a patient and their family when developing a healthcare plan. The Kansas Department of Health and Environment (KDHE) Title V Maternal and Child Health Services Program collaborated with the Kansas Health Institute in 2023 to investigate how the HCC model could be expanded across primary care and maternal and child health programs in Kansas. This report offers insights into the current HCC services available to Kansas residents who are pregnant, in their postpartum period, or children, particularly those aged 0-8. The goal is to guide future enhancements to HCC policy and practice throughout Kansas.

The HCC approach emphasizes the importance of addressing all aspects of a patient’s well-being, not just their physical health. By considering the emotional and social factors that can impact a patient’s health, HCC aims to provide comprehensive and individualized care that meets the unique needs of each patient and their family. This approach is particularly beneficial for pregnant women, new mothers, and young children, as it recognizes the importance of early intervention and support in promoting positive health outcomes.

Through their collaboration, KDHE and the Kansas Health Institute sought to evaluate the current state of HCC services in Kansas and identify areas for improvement. By collecting data and feedback from patients, providers, and stakeholders, they were able to assess the strengths and weaknesses of the current system and determine how HCC could be more effectively integrated into primary care and maternal and child health programs across the state. The findings of this study will inform future policy and practice decisions to enhance the quality and accessibility of HCC services for all Kansas residents.
